CHP Contracted Insurance Plans
Children's Health Partners accepts most major commercial insurance plans, as listed below. However, since your plan is a contract between you and your insurer, it's your responsibility to do the following:
- Verify with insurance company that CHP is included in your specific plan prior to first appointment
- Bring current insurance card to all visits
- Inform staff of any plan changes, even with same insurer
- Familiarize yourself with plan coverages and limitations (co-pays, in-network hospitals, specialists, laboratories, imaging facilities, pharmacies, etc.)
- Be prepared to pay co-pay at time of service
- Understand unpaid balances are billed to guarantor; parent seeking treatment is responsible for unpaid balances
