Tracking your child’s development is a key component of pediatric care. During every well child visit our doctors evaluate your child’s progress. As part of this process, we are now providing online developmental screening via the Ages and Stages, Third Edition (ASQ-3) program at the 9 month, 18 month and 30 month well child visits. The screening questionnaires are available at https://www.asqonline.com/family/39dd88. The link will also be available on our website. You can access it from your computer, tablet, or smartphone.
To get started, we ask that you go to the website no sooner than two weeks before your child’s 9 month, 18 month or 30 month birthdays and enter your child’s name, date of birth, and number of weeks premature, if any, and choose “I am screening my child today” and click on Submit. You will then be given the option to print the age appropriate questionnaire, review the questions, and work with your child before answering, if needed. Then, at least 3 days prior to your child’s 9, 18, or 30 month well child visit we ask that you complete every question in the questionnaire and submit it. (Once you start answering the questions, you will not be able to save and return, so allow yourself at least 15 minutes to complete it.)
The questionnaires include questions about your child’s communication, gross motor, fine motor, problem solving, and personal social skills; some of which your child may or may not be able to do. After you submit the questionnaire, CHP will be able to access the results online and you will be offered access to a document with age appropriate activities for you and your child.
The doctors will review the results with you at your child’s well visit. If the questionnaire shows that your child is developing without concerns, we will provide some ideas about how to continue to encourage your child’s development, and have you complete the next questionnaire at the appropriate time. If the questionnaire shows some possible concerns, we will provide the appropriate information and referrals.
